Scope

This policy describes how Insurazon handles information submitted through this website. The site owner must add the operating legal entity and mailing address before activating delivery to third-party lead buyers.

Information collected

Insurazon may collect your name, email address, telephone number, ZIP code, insurance category, contact preference, coverage information, notes you provide, referral parameters, and technical information needed to operate and protect the website.

Do not include Social Security numbers, payment card information, medical records, or other highly sensitive information in an open notes field.

How information is used

Information may be used to store and review your matching request, respond to support or privacy questions, maintain security, prevent abuse, and improve the website. Optional marketing messages from Insurazon require separate consent.

Analytics and local storage

Insurazon uses Google Analytics only after you choose Allow analytics. Google Analytics helps measure page visits and the steps people complete in the matching request. Insurazon does not send names, email addresses, telephone numbers, ZIP codes, coverage notes, or other insurance form field values to Google Analytics.

Your analytics choice is stored in your browser so the site can remember it. Global Privacy Control and Do Not Track keep optional analytics off. Analytics choices are available here without a pop-up. Google may process analytics data as described in its information about partner sites.

Sharing and insurance providers

The current matching request does not identify an insurance provider as a recipient. Insurazon will not treat a general request as permission to send your information to an unnamed provider. Before a provider receives personal information, the request flow must identify the recipient and present any disclosure and consent required for that transfer.

Information may also be disclosed to service providers that host, secure, or maintain the website, or when required by law. This policy does not claim that Insurazon never sells or shares information because the future lead-buyer model has not been finalized. The policy must be updated before that model is activated.

Retention and security

Insurazon uses technical safeguards intended to reduce unauthorized access. No online system is completely secure. A specific retention schedule must be approved and added before third-party lead delivery begins.

Your choices

You may request access, correction, or deletion, and may withdraw optional marketing consent, by emailing privacy@insurazon.com. Rights vary by location, and Insurazon will evaluate requests under applicable law.

Health and Medicare information

Insurazon is not the federal Medicare program and does not claim that information submitted here is protected by HIPAA. Avoid providing medical details unless a later form clearly explains why they are needed and who will receive them.
